The YMCA of Pierce and Kitsap Counties is a hub of connection, care, and community support. We offer a host of health and wellness programs for every age and stage of life, including evidence-based chronic disease programs, local community groups, and more. Recently, the YMCA Health Initiative Department secured funds to help extend vital services to Kitsap County via one-on-one Care Coordination.
Thanks to partnerships with Olympic Community of Health and other community-based organizations in the Olympic Region (Clallam, Jefferson, and Kitsap Counties), the Y will bring Care Coordination to community members through Olympic Connect.
The YMCA team of Care Coordinators knows that health is about so much more than a visit to a doctor’s office or clinic. It’s also about having access to safe and reliable housing, food security, employment opportunities, transportation options, family support, and social connections. YMCA Care Coordinators, known as trusted helpers, provide individuals with one-on-one support they need to achieve their health and life goals.
Olympic Connect makes it easier to surround community members with care by coordinating between health care and social service providers and tracking progress for healthier individuals and families. This partnership with Olympic Connect is another great way the YMCA continues to create opportunities for all to thrive in every season of life.
